It depends on the intensity of playing. On average, let's say once every six months, but sometimes guitarists change them before every concert.

The general “standard”, if you can call it that, is a caliber 45-105 for a four-string guitar. But you have to try other options to find the right one for you.

Wash your hands, wipe the strings after playing with a lint-free cloth (microfiber is fine), and it's a good idea to periodically wipe with a cleaning fluid.
